What is established (grade A/B)

  • Alpha2delta ligands and gabapentin enacarbil are the best-supported non-dopaminergic drug class, with equivalent efficacy to rotigotine and pregabalin shown in a comparative network meta-analysis of 7,333 patients (2017-iftikhar-network-meta-analysis-alpha2delta.md, grade A) and confirmed superior to pramipexole on augmentation risk in the pivotal 719-patient head-to-head RCT (2014-allen-nejm-pregabalin-pramipexole-augmentation.md, grade B). Two 2026 model-based network meta-analyses of gabapentin enacarbil versus pregabalin (2026-iftikhar-placebo-nocebo-network-metaanalysis.md; 2026-iftikhar-benefit-risk-dose-response-metaanalysis.md, both grade A) quantify a narrow therapeutic index for both drugs (TD50/ED50 approximately 1.0-1.2) and a large (40%) placebo-responder rate in RCTs of this class - important context for interpreting any RLS trial.
  • Tonic motor activation (TOMAC) is the most rigorously validated device in the pipeline. A 133-patient pivotal RCT (2023-bogan-tomac-restful-rct.md, grade B), a 45-patient trial in both medication-naive and refractory patients (2024-singh-tomac-naive-refractory-rct.md, grade B), a 320-patient meta-analysis (2025-mohamed-tomac-meta-analysis.md, grade A), and a 252-patient individual-participant-data meta-analysis (2026-karroum-tomac-individual-participant-meta-analysis.md, grade A) together make TOMAC the only device in this pipeline with grade-A pooled evidence.
  • The 2025 AASM guideline (2025-winkelman-aasm-guideline-rls.md, grade D but built on the above grade A/B evidence) formalizes alpha2delta ligands and IV iron as first-line, and downgrades dopamine agonists to conditional/second-line because of augmentation - the framework this entire pipeline synthesis is organized around.
  • Prolonged-release oxycodone-naloxone has grade-B RCT evidence specifically for treatment-refractory RLS (2013-trenkwalder-oxycodone-naloxone-rct.md, already in this knowledge base), with mu-opioid receptor Ki around 12 nM confirmed via ChEMBL (see pharmacology/oxycodone.md).

Emerging C

  • Crisugabalin (HSK16149) is a genuinely new alpha2delta calcium-channel ligand now in an early-phase RLS trial (2026-beijing-friendship-crisugabalin-phase1-trial.md), backed by two grade-B human RCTs in adjacent indications - postoperative pain (2026-yang-crisugabalin-postoperative-pain-trial.md) and diabetic peripheral neuropathic pain (2026-fu-crisugabalin-diabetic-neuropathy-trial.md) - that establish its alpha2delta mechanism, tolerability, and sleep-interference benefit in humans, but not yet in RLS itself.
  • TOMAC real-world and mechanistic data continue to accumulate at grade C: a 24-week open-label extension (2023-roy-tomac-open-label-extension.md), an opioid-dose-reduction pilot (2023-buchfuhrer-tomac-opioid-dose-reduction.md), extended-duration use for nocturnal awakenings (2026-alawieh-extended-duration-tomac-rls.md), acute polysomnographic effects (2026-rigot-tomac-acute-sleep-polysomnography.md), and a 325-patient real-world THRIVE observational study (2026-singh-tomac-thrive-real-world.md).
  • Spinal cord stimulation (SCS) has scoping-review-level support (2023-pagani-estevez-scs-scoping-review.md; broader 2026-thrash-neuromodulation-systematic-review.md, both grade C) but only case-series-level primary data until the sham-controlled REST-SCS crossover trial now registered (2026-ars-medica-burst-scs-sham-trial.md) and a parallel mechanistic electrophysiology study (2025-uab-spinal-cord-stimulator-mechanism-trial.md) report results.
  • Dipyridamole (ENT1 inhibitor, adenosine hypothesis) has one grade-B crossover RCT (2021-garcia-borreguero-dipyridamole-rct.md) and a grade-C open-label pilot (2018-garcia-borreguero-open-label-dipyridamole.md); its ChEMBL-confirmed pharmacology (Ki approximately 8 nM at ENT1, no direct A2a affinity - see pharmacology/dipyridamole.md) clarifies it works indirectly by raising extracellular adenosine, not by binding an adenosine receptor.
  • Peroneal/tibial nerve stimulation devices in special populations, e.g. the pregnancy-specific TOMAC feasibility pilot (2026-rhode-island-pens-pregnancy-trial.md), remain grade E pending results.
  • Non-pharmacological augmentation biomarkers, e.g. a nasal-swab DRD2 gene-expression study (2025-hbc-immunology-drd2-augmentation-biomarker.md), are conceptually promising but entirely unvalidated E.

Contested or negative findings

  • Suvorexant (orexin antagonist) improved motor/sensory RLS symptoms and self-reported sleep in a 41-patient crossover proof-of-concept (2024-garcia-borreguero-suvorexant-rct.md, grade B), but a companion 34-patient RCT found improvement in self-reported, not actigraphic (objective), sleep measures (2024-winkelman-suvorexant-actigraphy-null.md, grade B) - a clear subjective/objective discordance that should temper enthusiasm.
  • Pregabalin failed to separate from placebo in a 78-patient Korean RCT (2025-sunwoo-pregabalin-korean-negative-rct.md, grade B), contrasting with pregabalin's otherwise strong evidence base (above) and illustrating population/ethnicity-dependent variability in trial outcomes.
  • Transcranial direct current stimulation (tDCS) showed no effect in a 33-patient proof-of-concept RCT in drug-naive RLS (2014-koo-tdcs-negative-rct.md, grade B) - a clean negative result for one non-invasive neuromodulation modality, distinct from the more positive (but lower-grade) signal for spinal cord stimulation.
  • Non-pharmacological interventions in hemodialysis-associated RLS show large point estimates for aromatherapy-reflexology and stretching in a network meta-analysis (2026-al-harbi-hemodialysis-nonpharm-network-metaanalysis.md), but the authors themselves grade the certainty "very low," note the entire evidence base is single-country (Iran), and call the result hypothesis-generating only.
  • Istradefylline (adenosine A2a antagonist) - the pharmacological opposite of dipyridamole's adenosine-boosting approach - showed benefit in 3 of 5 patients in a 2007 pilot (2007-decerce-istradefylline-open-label-pilot.md, grade C) but worsened insomnia in 2 of those 5, and a single 2018 case report in Parkinson's-associated RLS (2018-nuermaimaiti-istradefylline-case-report.md, grade E) is the only other human evidence found; the drug was never developed further for RLS, leaving genuinely conflicting signals about which direction of adenosine-system modulation helps versus harms RLS/sleep.

Mechanistic picture in 5 bullet points

  • Dopamine agonists act through D2-like receptors with markedly different subtype selectivity: pramipexole is strongly D3-preferring (Ki approximately 1 nM at D3 vs approximately 21-27 nM at D2, negligible D1), ropinirole is roughly balanced D2/D3 (both low single-digit-to-low-double-digit nanomolar), and rotigotine is the most potent at D2 (sub-nanomolar in the high-affinity state) while also binding D3 potently (Ki approximately 4 nM) - see pharmacology/pramipexole.md, ropinirole.md, rotigotine.md.
  • Alpha2delta ligands (gabapentin, gabapentin enacarbil, pregabalin, and now crisugabalin) all act by binding the alpha2delta-1 auxiliary subunit of voltage-gated calcium channels, a mechanism first established by 1996-gee-alpha2delta-gabapentin-binding.md and confirmed for each drug via FDA label mechanism-of-action text, reducing presynaptic calcium influx and downstream (likely glutamatergic) neurotransmitter release rather than acting on dopamine receptors at all.
  • The adenosine system can be targeted in two opposite directions with apparently opposite consequences for sleep: dipyridamole indirectly raises extracellular adenosine by blocking its cellular reuptake (ENT1 inhibition, Ki approximately 8 nM, no direct A2a affinity), while istradefylline directly blocks the A2a receptor - and pilot human data exist for both approaches, with neither yet confirmed in an adequately powered trial.
  • Opioids (oxycodone, mu-opioid Ki approximately 12 nM) are believed to act partly by modulating the same A11 diencephalospinal dopaminergic pathway and spinal excitability implicated in RLS pathophysiology, rather than purely through classical analgesic mechanisms, making them mechanistically distinct from both the dopaminergic and alpha2delta drug classes.
  • Kinetic profile (not just receptor target) plausibly shapes augmentation risk: levodopa's very short half-life (~50 minutes free, ~1.5 hours with carbidopa) produces the sharpest, most pulsatile dopaminergic exposure and carries the highest augmentation rate, oral pramipexole/ropinirole produce an intermediate peak-and-decline profile over 6-12 hours, and transdermal rotigotine's continuous delivery is hypothesized (not yet directly tested in this literature) to produce more stable receptor occupancy - a testable hypothesis for the simulation team.

Implications for a non-drug long-term plan for people currently on dopamine agonists or alpha-2-delta ligands

  • The pipeline data support what the AASM 2025 guideline already recommends: alpha2delta ligands are the appropriate default long-term drug for patients tapering off a dopamine agonist, but the 2026 dose-response meta-analyses show their own therapeutic index is narrow (TD50/ED50 approximately 1.0-1.2), so a "just increase the alpha2delta dose" strategy will hit dose-limiting somnolence/dizziness quickly - individualized, slow titration matters as much for this class as avoiding augmentation does for dopamine agonists.
  • TOMAC (grade-A pooled evidence) and, with more caution, spinal cord stimulation and peroneal/tibial nerve stimulation (grade C, controlled data pending) are the most trial-supported non-drug, non-opioid options to offer as a bridge or adjunct during a dopamine-agonist taper, particularly for patients seeking to reduce or avoid an opioid bridge (see 2023-buchfuhrer-tomac-opioid-dose-reduction.md for direct evidence of opioid-sparing).
  • Because the large (approximately 40%) placebo response documented in alpha2delta-ligand trials applies equally to how any new non-drug intervention will look in short, unblinded, self-reported trials, early symptom improvement after starting ANY new device or supplement (in a clinic or from patient reports) should be interpreted cautiously and not treated as proof of a specific mechanism until confirmed in a sham-controlled design.
  • For patients on dopamine agonists who cannot tolerate or do not respond to alpha2delta ligands, opioids remain the next evidence-based rescue option B rather than an unproven pipeline drug; none of the genuinely novel pipeline candidates identified here (crisugabalin, dipyridamole, istradefylline, SCS) yet has RLS-specific evidence strong enough to substitute for this established sequence.
  • Dopamine-agonist withdrawal itself must remain physician-supervised and gradual regardless of which non-drug or alternative-drug option is chosen; nothing in this pipeline review changes that safety requirement (see tapering-deprescribing.md and 2026-martin-garcia-structured-dopamine-agonist-withdrawal.md, already in this knowledge base).

Open questions

  • Does a numeric alpha2delta-1 binding affinity (Ki/Kd) for gabapentin or pregabalin exist in a peer-reviewed, primary source that can be directly verified (ChEMBL's curated bioactivity database returned no quantitative records for either drug against the relevant calcium-channel target, despite confirming the qualitative mechanism) - this is a genuine gap in the otherwise strong mechanistic case for this drug class.
  • Does relative D2 versus D3 receptor selectivity (pramipexole's strong D3 preference vs ropinirole's balanced D2/D3 vs rotigotine's high potency at both) predict differential augmentation risk or rate between the three approved dopamine agonists? No study directly comparing this was found in this search.
  • Will the crisugabalin RLS trial (2026-beijing-friendship-crisugabalin-phase1-trial.md) or the sham-controlled SCS trial (2026-ars-medica-burst-scs-sham-trial.md) report positive results, and if so, at what point will either be validated in a properly powered, placebo/sham-controlled trial given the large placebo response documented in this drug class?
  • Can the nasal-swab DRD2 gene-expression approach (2025-hbc-immunology-drd2-augmentation-biomarker.md) or the objective Suggested Immobilization Test with heart-rate-variability analysis (2025-siriraj-immobilization-test-hrv-trial.md) mature into validated biomarkers for augmentation risk or diagnostic accuracy, respectively - both are conceptually attractive but currently unvalidated E?
  • Which direction of adenosine-system modulation (ENT1 inhibition/reuptake blockade as with dipyridamole, versus direct A2a antagonism as with istradefylline) is actually beneficial for RLS motor symptoms without worsening sleep, and could a trial ever adequately compare both mechanisms head-to-head?
  • Given the IRLSSG's own diagnosis that trial numbers and industry interest in RLS drug development are declining (2024-garcia-borreguero-clinical-trials-roadmap.md), will device-based and repurposed-drug approaches (TOMAC, SCS, crisugabalin, dipyridamole) increasingly substitute for genuinely novel RLS-specific drug development in the coming years?
