Diese Seite versammelt die zugrunde liegenden klinischen Leitlinien, erklärt, wie die zugrunde liegende Forschung gesammelt und geprüft wurde, wie das langfristige Protokoll erstellt und gegengeprüft wurde, was die Simulation ist (und nicht ist), den vollständigen medizinischen Haftungsausschluss und die bekannten Grenzen dieses Projekts.

Ein Vergleich nationaler und internationaler klinischer RLS-Leitlinien — Diagnose- und Behandlungsalgorithmen aus den USA, Europa, Deutschland, Frankreich und internationalen Gremien (IRLSSG/EURLSSG, World Sleep Society) — sowie die historische Entwicklung der US-Leitlinien von 2004 bis 2026. Dieser Vergleich konnte keine bestätigte, textextrahierbare Primärquelle für die aktuelle spanische oder vollständige japanische Leitlinie finden und fand keine eigenständige, indizierte nationale RLS-Leitlinie für Kanada, Australien, China oder Indien — diese Lücken sind vermerkt, nicht mit einer Vermutung gefüllt (guidelines-global.md, „Offene Fragen“). Getrennt davon fand eine World-Sleep-Society-Umfrage 2025 unter 53 nationalen Schlafgesellschaften (23 Antworten), dass 12 der 20 Empfehlungen der AASM-Leitlinie 2025 international vollständig und 5 teilweise unterstützt wurden, wobei die beiden Empfehlungen gegen Dopaminagonisten der wichtigste regionale Streitpunkt waren (C, 2025-ferri-world-sleep-society-endorsement.md).
2 · Methodik
Wie die Wissensdatenbank aufgebaut wurde
Eine Forschungswissensdatenbank mit einer Markdown-Notiz pro Quelle, aufgebaut in zwei parallelen Durchgängen und vor jeder Veröffentlichung hier auf Zitationsgenauigkeit geprüft.
Ein Themendurchgang erbrachte 22 Prosa-Synthesen — eine pro Themenbereich (Eisentherapie, Augmentation durch Dopaminagonisten, Ausschleichen und Deprescribing, Genetik und Epidemiologie, Leitlinienvergleich über Länder hinweg und 17 weitere) —, die jeweils die einzelnen dahinterstehenden Studiennotizen zitieren. Ein Sprachdurchgang durchsuchte 25 nicht-englischsprachige Sprachgruppen (Arabisch, Tschechisch/Slowakisch, Deutsch, Griechisch, Spanisch, Persisch, Finnisch, Französisch, Hebräisch, Hindi, Ungarisch, Indonesisch/Malaiisch, Italienisch, Japanisch, Koreanisch, Niederländisch, Polnisch, Portugiesisch, Rumänisch, Russisch, Skandinavisch (Schwedisch/Norwegisch/Dänisch), Türkisch, Ukrainisch, Vietnamesisch/Thailändisch und Chinesisch) nach nicht-englischen Befunden, nationalen Leitlinien und Patientenorganisationen; derzeit existieren verifizierte Einträge in 11 dieser Sprachen — die meisten Durchgänge, die nichts Zitierfähiges fanden, sind als solche vermerkt statt stillschweigend verworfen.
Jede Notiz — eine pro Studie, Studienreihe oder Leitlinie — folgt derselben Vorlage aus Frontmatter und Abschnitten: eine stabile ID, Titel, Autoren, Jahr, Zeitschrift, eine PMID/DOI/URL, Ausgangssprache, Studientyp, Stichprobengröße, Population, kontrollierte Themen-Tags und ein Evidenzgrad, gefolgt von einem Kernbefund, der Intervention oder Exposition, Ergebniszahlen, der Relevanz für einen nicht-medikamentösen langfristigen Plan und Limitationen (gemäß der Notizvorlage in docs/00_MASTER_PLAN.md §5).
Jede sachliche Aussage in der Wissensdatenbank, im Protokoll und auf dieser Website wird nach derselben A–E-Skala bewertet:

3 · Methodik
Wie das Protokoll erstellt wurde
Das langfristige Protokoll durchlief vor der Veröffentlichung eine unabhängige Erstfassung, Beurteilung, gegnerische Zitationsprüfung und eine separate Redlichkeitsprüfung.
Das langfristige Protokoll wurde in drei unabhängigen Entwürfen erstellt — risikoorientiert, evidenzorientiert und patientenorientiert —, jeder separat aus derselben Wissensdatenbank verfasst, dann von einem unabhängigen Beurteilungsdurchgang überprüft und zu einem einzigen Dokument zusammengeführt (docs/00_MASTER_PLAN.md §6, Phase R2-B). Danach wurde jede der 40 tragenden Aussagen einzeln gegen die zitierte Notiz der Wissensdatenbank, den PubMed-Eintrag zu ihrer PMID und jede widersprechende Notiz an anderer Stelle in der Wissensdatenbank neu geprüft — ein gegnerischer Verifizierungsdurchgang, gefolgt von einer separaten Zeile-für-Zeile-Redlichkeitsprüfung gegen die eigenen unverhandelbaren Regeln des Projekts, einschließlich einer vollständigen Prüfung auf die Existenz jeder Quellenangabe und einer stichprobenartigen Zahlenprüfung von 15 zufälligen Notizen.
Ergebnis: 32 der 40 Aussagen blieben unverändert bestehen. Sieben wurden umformuliert, weil sie über ihre Quelle hinausgingen — ein Prozentsatz, der über die Population hinaus verallgemeinert wurde, in der er gemessen wurde, eine kleine Fallserie, die bewertet wurde, als wäre sie eine große Studie, ein gepoolter Effektwert, der dargestellt wurde, ohne zu erwähnen, dass er von einer anderen Patientenpopulation getrieben war. Eine Zahl — eine an anderer Stelle vielfach wiederholte Ausschleichrate — wurde vollständig gestrichen, weil sie in keiner Quelle dieser Wissensdatenbank enthalten ist.

Scope: all 40 load-bearing claims (C01–C40) in docs/SOLUTION_PROTOCOL.md were independently
re-checked against (a) the cited knowledge-base note, (b) the PubMed record for the cited PMID, and
(c) any contradicting note elsewhere in knowledge-base/. Evidence grades were re-checked against
the rubric in docs/00_MASTER_PLAN.md §2. Verdicts below are the reviewers'; the "Action taken"
column is this revision.
Verdict counts: 32 supported, 7 overstated, 1 wrong-citation. Eleven claims were rewritten, three regraded, one numeric figure deleted outright as untraceable.

Disagreements with the reviewers
None material. Two reviewer suggestions were declined and are recorded here rather than silently dropped:
- C04 — the reviewer offered an optional downgrade of the IRLS MCSC letter from C to D. Kept at C: it is a secondary statistical analysis of a real n=196 validation cohort, its limitations are already printed on the note, and the 6-point threshold is used in this document only as a conservative brake on claiming benefit, so an error in either direction is non-inflating.
- C18 — the reviewer asked for a caveat that the vitamin-D systematic review is not independent of the Wali RCT. The claim it supports is negative ("do not treat RLS with vitamin D"), so non-independence cannot inflate it; recorded here instead of adding text.
Follow-ups outside this document
- KB grade drift. Three notes carry frontmatter grades that this review contradicts:
2008-rottach-second-generation-antidepressants-rls.md(B, should be C),2010-whittom-melatonin-bright-light-rls.md(B, should be C, and its ownstudy_typealready sayscase-series), and2018-wolf-ferric-carboxymaltose-hypophosphatemia-rct.md(A, should be B — single RCT). The protocol now uses the corrected grades. The notes andcatalog.jsonwere not edited here, becausecatalog.jsonis generated and editing the notes alone would desync the index; this needs a single pass that regrades the notes and rebuilds the catalog together. - Citation-checking path bug.
2014-allen-irlssg-updated-diagnostic-criteria.mdand the three iron-threshold guideline notes live inknowledge-base/guidelines/, notknowledge-base/studies/. Any automated citation checker that searches onlystudies/will report these as missing. Fix the checker's search path, not the files.
Summary
Forty claims were re-verified against their notes and their PubMed records. Thirty-two survived unchanged in substance. The failures cluster into three recognisable species. First, quantifiers that drifted past their source: "at far higher doses" (C03), a 7.1% ICD rate silently narrowed to agonist-treated patients when the denominator included levodopa (C07), an H2-antagonist association generalised to a cohort where it was frankly null (C21), and a pooled temperature-therapy effect sold to idiopathic-RLS readers when it was driven by haemodialysis patients (C39). Second, grade inflation against the project's own rubric: two observational series and one single RCT carrying grades reserved for randomised or pooled evidence (C20, C22, C30) — and, tellingly, the same inflation sits in the knowledge-base notes themselves, so this is systemic rather than a transcription slip. Third, and most serious, one number with a citation that does not contain it: the "10–25% every 2–4 weeks" taper rate (C35), attributed to a narrative review that gives no numeric schedule at all. That figure has been removed rather than re-sourced, because no source for it exists anywhere in this knowledge base; the document now says so in plain words, in the one section where a false precision would have been most dangerous.
The net effect of this revision is that the protocol claims less than it did, and says where it is guessing. Nothing in the therapeutic sequence changed — iron first, add before you subtract, taper only under supervision, judge nothing before twelve weeks — but the confidence attached to several of its supporting numbers is now lower and, in the taper-rate case, explicitly absent. That is the correct direction of travel for a document whose readers may act on it.
4 · Modell
Über die Simulation
Ein mechanistisches Modell des auf dieser Website beschriebenen Mechanismus — kein Vorhersagewerkzeug dafür, was bei einer bestimmten Person geschehen wird.
Eine separate mechanistische Simulation modelliert Eisenabbau, Dopaminsignalgebung und das Absetzen von Dopaminagonisten im Zeitverlauf. Sie ist ausdrücklich ein mechanistisches Modell, kein Vorhersagewerkzeug: Nach ihrer eigenen Spezifikation kann sie nicht vorhersagen, was bei einer bestimmten Person geschehen wird, trägt keine patientenspezifischen Kalibrierungsdaten, und ihre Symptomindex-Ausgabe (0–10) ist nur locker auf einen echten IRLS-Wert abgebildet — ein Unterschied von weniger als etwa einem Punkt ist nicht bedeutsam. Jede von ihr erzeugte Zahl und jeder Datenexport trägt dasselbe Etikett: „Mechanistisches Modellergebnis — keine klinische Vorhersage.“

6 · Grenzen
Grenzen, und wie man einen Fehler meldet
Bekannte Lücken dieses Projekts, offen benannt, sowie wie man einen Fehler meldet.
- Dieses Projekt konnte die konkreten Zahlenschwellen in der aktuellen spanischen (SEN/SES) oder vollständigen japanischen nationalen Leitlinie nicht bestätigen — das spanische Dokument war ein nicht lesbarer Bildscan, und die japanische Leitlinie war nur über eine sekundäre narrative Übersichtsarbeit zugänglich — und fand keine eigenständige, indizierte nationale RLS-Leitlinie für Kanada, Australien, China oder Indien. Diese Lücken sind vermerkt, nicht mit einer Vermutung gefüllt.
- Eine Notiz der Wissensdatenbank bleibt nicht verifizierbar — ihre Quellenangabe konnte gegen keine indizierte bibliografische Datenbank bestätigt werden — und ist bis auf Weiteres vom öffentlichen Katalog und vom Protokoll ausgeschlossen.
- Die letzte vollständige Zitationsverifizierung lief am 2026-09-12; dieser Katalog wurde zuletzt am 2026-09-15 neu erstellt. Nach diesen Daten veröffentlichte Forschung ist hier noch nicht berücksichtigt.
- Drei Notizen der Wissensdatenbank tragen einen im Frontmatter angegebenen Evidenzgrad, den die Verifizierung als zu hoch beurteilte; das Protokoll verwendet bereits den korrigierten Grad, aber die Notizen selbst und der generierte Katalog wurden noch nicht entsprechend neu bewertet (siehe das Verifizierungsprotokoll oben, „Follow-ups außerhalb dieses Dokuments“).
- Die Gehirn-und-Dopamin-Simulation ist mechanistisch, nicht vorhersagend — siehe „Über die Simulation“ oben.
